Five hours on a boat, and only for half the year. Outside the season the answer is to go overland to Pak Bara in Satun and cross from there.

Pak Bara pier in Satun province is the reliable way in. It is a long way south of Phuket, so this is the option for people already heading down the coast.
The season changes this. Koh Lipe is the most seasonal destination in this table. The direct boat from Phuket does not run in the southwest monsoon at all, and much of the island closes with it.
These times and fares come from booking agents and route aggregators, not from operator timetables. Most Thai ferry and minivan operators publish no English schedule.
